Sadia A. Ashak-Kuzmickus, 54, of Libertyville, IL passed away on Wednesday, November 12, 2008 at Westmoreland Extended Care in Lake Forest, IL. She was born July 1, 1954 in Iraq and married Frank Kuzmickus on July 27, 1995. She was an Associate Chemist at Abbott Labs at Abbott Park for over 20 years and was a member of St. John's Syrian Orthodox Church in Villa Park, IL.

She is survived by her husband, Frank; daughter, Dollie Kuzmickus at home; four sisters, Muna Sielauf of Brussels, Belgium, Hanan Ishak of Germany, Miluk Ishak of Sweden and Tafha Ishak of Sweden; three brothers, Daniel, Sahd and Rahd Ishak all of Sweden.
She was preceded in death by her parents and one brother in infancy.
Visitation will be held on Sunday, November 16, 2008 from 4-8pm at the Salata - Gurnee Funeral Home, 4190 Old Grand Avenue, Gurnee, IL and on Monday, November 17, 2008 from 9-10am at Holy Family Church, 450 Keller Avenue, Waukegan. Services will be held at 10:00am at the church. Interment to follow at Ascension Cemetery in Libertyville, IL.
